Before completing the application form, please ensure that you are NOT logged into your IPEM account (if you have one). It is also recommended that you copy your responses into a word document as you complete the form, as a precaution. If you receive an error message when submitting your application, in most cases the application will still come through to us, but we would recommend that you email training@ipem.ac.uk to check that it has been received. We are working to fix this issue.

PLEASE NOTE - The online form cannot be saved and completed in stages. It is a large form, so you have the option to download the word version instead.

You are required to upload the following documentation as part of your application. You will need to have this information ready prior to filling out the application form:

1. Department/Consortium organisation chart
2. Training plan for each subject area to be accredited. (The official IPEM template for Medical Engineering and Nuclear Medicine training plans must be used. Official templates for the other disciplines will be available soon).
3. Major subject areas document (if accrediting at more than one hospital)
4. List of proposed External Moderators (please read the role description for eligibility requirements)
